I'd say off the top of my head from the games I played...

Shadow of the Colossus
Timesplitters: Future Perfect (Well, everything in THAT game was a spoiler)
Metroid didn't really surprise me that much

There ARE more but I can't remember the others...

Oh!

Also, one of my favorite flash game endings comes from "Level Up!" (you can find it on Newgrounds). You can only get the surprise ending when you get 100% completion =)

Star Ocean 3, some people thought it was bad and I definitely felt the pain but it was still a pretty big plot twist, felt weird and I call that a good plot twist if it was able to make me feel like doodoo

Dragon Age Origins, the betrayal at the battle of Ostagar. I kinda expected it but it's was still a nice twist.
