Btw, how to use that attachment for converting to a GPA 4.0 scale?

Just enter the number of credits for each grade. It's not the official converter though. Some institutions or global applications use their own conversion tables.

I understand that a 10=3.7 and 11=3.9, but what if your average is in between those two, let's say 10.7. Does that mean it's 3.8?

You convert every grade to the 4.0 scale before calculating your average.

on the back of transcrip states the how 12 grading scale converted to the percentage grading scale

This is another option for anyone interested in keeping track of their marks.
It uses the mcmaster email account (google account) to log you in like you can use google to log into various websites.
