Parent Information Form
I was just going over the Getting Registered Checklist and found a "Complete and submit Parent Information Form" check box. What IS the parent information form?? and where can i find it?

The parent information form is a...form (I don't know what else to call it!) that asks for all of the contact info etc for your parents. It's basically how the school will get ahold of your family in case something goes wrong.

sounds like something you should already have from your offer of admission package, cause you can't download it online

It isn't listed in their list of forms:
http://registrar.mcmaster.ca /registered/forms.html

So its something you probably already have sitting around somewhere

If you don't have it, and it isn't available online, then I wouldn't worry about it for now. Try calling them up when you get a chance to double check, and let us know how it goes

im not sure if this is what you are talking about but when you log into mugsi click on "My Profile" and in the quick links you can go to emergency contacts and add them there

so i emailed a liaison officer, asking where i could find this parent information form, and this is what they said:

"Hi Ashley,

Thank you for using MacIQ! We should not be using the parent information form for September 2008 admission cycle.

If a specific McMaster department is requesting that document from you, please contact that department for a copy of the Parent Information Form.

Best Wishes."

so there you have it

The Parent Information Form is found here:

You submit it online when you've completed it.

Found it from the 2008/2009 Fall/Winter Registration Dates and Deadlines page:

To select your faculty, it's in the upper right hand corner of the page. Be sure to pick if you're full or part time as well (top middle of the page, below the title).


However, like Chad said, double check to make sure you have to submit it. But even if you do submit it, i guess they can always disregard it.

If you read a few replies back it appears that the MacIQ service has said the form is no longer necessary.

Hi..I was just wondering if all first year students have to complete the parent information form...Do international students have to complete the form as well?

This form is NOT mandatory for first years. I emailed the registrars office and they confirmed this for me.
