What I'm wondering is how to format this for shipping from an online store, like Amazon.com.
You're supposed to divide things into 'Address line 1' (Street address, P.O. box, company name, c/o) and 'Address line 2' (Apartment, suite, unit, building, floor, etc), and I'm not sure what should go where. Anybody done this successfully?

That order you have is correct. Maybe include more than one of the lines into address lines 1 and 2. I don't think it really matters the order, as long as ll the information is there.
