First time using msaf for midterm..
First time using the msaf for a midterm, do I need to explain why I am using it? I know you are allowed to use one form per term, is that without any reason/medical documentation?

My midterm is tomorrow night, should it be submitted sometime tomorrow or following the midterm?

fill that out with whatever excuse you wish. email whoever it says to email in the course outline telling them that you msafed.

It doesn't matter. As long as you do it within 5 days after the midterm.

I just missed the midterm... Thinking of doing the msaf. I heard you have to include your instructors email address, would that be my current professor or would that be the course coordinator?

Assuming you are talking about Bio, I used the MSAF last term for Bio 1M03, you have to email Alastair Tracey, he is the one that is responsible for grades. You have to email him or else you will get ZERO!

Thinking? Hah, why wouldn't you MSAF? Either way, like the previous poster mentioned, you should make an effort to inform the professor or coordinator of the course to make sure they know you MSAF'd. McMaster (or whatever service) auto emails them, usually, but some professors are stringent about being contacted directly.

Yeah haha of course I ended up using the MSAF, I just meant I was thinking of getting it done tonight, which I just did. I sent it through, should I be following up and stopping by his office hours on Monday to confirm the MSAF?

You probably should, especially if you were sick or intend on doing an alternate write (always a better choice than shifting mark to exam weight, but meh). If you don't have a really good reason, you of course have the right to say it was for 'personal reasons' and leave it at that. At the very least, email, and go in if theres no response ("hey I msaf'd, should i come in and see you, blah blah").

Yes, it's your responsibility to find out what will happen next in terms of the missing percentage.

It's fine to MSAF even the day after. There is a drop-down option which you enter as your reason for using MSAF. Your one use per term doesn't require any documentation to prove your reason. But always contact your prof to make sure they've received your MSAF (I have once submitted one that my prof did not receive, so I had to follow-up) and find out how it will be re-weighted or when it will be re-written.
