I need to MSAF a midterm worth 10% since I am sick. I have never used it before. Just wondering what sorts of questions are asked . For instance, does it ask for a reason and if so do I just say I was sick or a detailed explanation along with a doctor's note.

The point of the MSAF is that you don't need a note/documentation. Just go through the online form.
