I have a quick question. my car insurance company is giving me a discount if my average in first term is above 70%. and they need a transcript showing my grades. so my question is, where can I go to request such a transcript?

They have some tables to the left of the office with blue forms. fill it out.

Please note that they are printed off January 12th. Outside the Registrars office the blue forms are the Transcript Request forms as lorend indicated, if you are not deliviring it yourself you need to wait roughly 7-10 days after that day to get it mailed.
