Lost my T4, income tax papers..
How should I proceed? My father needs my T4 for the information on it to file for income tax. I lived in res this past year and received it through the mailbox at residence and accidentally lost it and I need to get this done, some of this is involved in the OSAP application I think which I need to do within the next week.

Should I call McMaster and ask them to scan the form for me and send through e-mail or something like that? What department should I contact? Is there another alternative?

Never been in this situation before, help would be appreciated.

Not sure about OSAP, but i think it should still be there on mugsi --> Tax certificates

lemme know if tht was it

T4 as in the one from your employer about the sum of your income? you can ask you employer and CRA

The T4 form for 2012 I received after not having worked at all during the year, the last time I had worked was June of 2011, I'm not even sure if my previous employer had me on their payroll or not, I haven't contacted them since.

You don't get T4 slips via MUGSI, only the T2202A.

The tax forms you do now are for 2011. Just contact your employer probably faster
