Fill out the form when you are better and contact your professor. If it's longer than 5 days you'll need a doctor's note.

Actually, they have recently changed the rules. You cannot use it on things worth 30% anymore.

Can I get a source on that? Didn't see that listed anywhere on the MSAF website.

Call the office of the associate dean of your faculty for questions about MSAF.

Oh really? So what's the cut-off? Or does that just apply to the summer :/ ?

are you sure? my prof for a course I'm taking right now told the class that they couldn't use it for the midterm (which was worth 30%). She seemed pretty firm about it.

Deferring a midterm worth 30% is a very difficult situation to be in. The final exam becomes weighted immensely high, and more than the material, it often shakes people's confidences in their abilities to test well

I for one, was surprised with how easy it was to get an MSAF, so It was probably abused a lot since it started. It makes sense that they would restrict it a little more.

It also makes life a lot more difficult for students who have ongoing medical illnesses that experience complications, student with disabilities and those individuals who are in very straining personal situations (i.e. family problems, etc) when other people abuse the MSAF. It requires them to get a whole lot more unnecessary documentation and be bounced around from one office to the next to ensure that their claims are still credible.

That's true, but I'm sure it wouldn't be very difficult for these individuals to provide the appropriate documentation required once any work is missed.
