I was wondering if anyone could please help me out on how to notify OSAP that I am indeed returning to mac? I have been told there is a form to fill out and hand in to mcmaster but I can't seem to find it.

Thanks in advance
I think you can find it online, no idea where though. The form is also located outside the SFAS office, I think. If it's not you can just go inside and ask for the form.

Go to your school financial aid office and speak to them.
