Some medical schools only look at your two highest year or two latest years when it comes to marks. Some also drop your lowest full year course for every year you've done if you've taken a full course load every year (U of T). So it really does depend. Research the schools you're interested in and find out!

The program you were looking at would give you two degrees, a PhD and a MD.

Yep! The MD/PhD program includes both an MD degree and a PhD, not just medical school aka. undergrad medical degree.

So when you're applying to med school in third year, do only your first year and second year marks count or do the med schools wait for your third year marks before they make a decision?

They see the thirs year term marks anyway, since they check them before your interview, which is usually at the start of the year (e.g. Feb or even later) And they take a look at your final marks afterwards, I believe. They take into account all your marks, so they would have to see your third-year marks sooner or later!

So then for schools like Western and Queens which take the top 2 years, your third year could potentially be one even if your applying in third year?

Yeah, but for Western you need 4 years of university before getting in. So even if you got accepted, you'd have to finish your 4th year with a minimum 3.7 GPA.
Queens and Western use GPA as a cut-off for interviews. Apparently any higher doesn't really make a difference.
My article has a bit more detail on grading and academic requirements: http://www.macinsiders.com/showthread.php?t=2191 6

summer school credits
Do all universities take summer school credit marks into account?
I heard some universities don't even look at the summer school courses, or was it only for the states?

each university is different. Some acknowledge it and won't include it in your overall GPA. Others don't care.. and treat it equally.

Yep, exactly, I remember a few schools didn't include it in the GPA, and it was just shown as a completion mark. But most schools do consider it. You just have to look at the policies for the different schools you're thinking of
