Hi, I know this is kind of a dumb question but I need to submit the Special Accommodation Request Form by 4pm tomorrow.

It says on the form that the 'forms may be faxed, mailed or hand delivered'. Since I don't have time to mail the form to them or hand deliver it, can I scan the form and email it to [email protected] r.ca? I'm asking this because they have a strict policy on fraudulent documents and they might not accept forms that are scanned because there's the possibility that you could digitally alter the 'professional attestation'; section?!

Has anyone who emailed the form, gotten acknowledgement of receipt from McMaster Housing yet?

It would be better to fax it since the instructions don't specify that you can email it. Just to be safe!

Do you know where I can fax it from and how much it'll cost? I honestly don't know how fax machines work.

Most convenience stores fax documents. It's very cheap costs less than $5 for sure. I remember places faxing for even $1 a page.

Community centers, copy centers, public library, staples (I think). I've see places for like 25 cents a page with nothing more than 1.50.

For future reference, you can email your special accommodation form request to [email protected] r.ca. I scanned my form and personal letter, combined them into a single pdf file and emailed them to the address. In the morning, I got an acknowledgement email that they received my form. Hope this helps future students. It's a lot more convenient to email them rather than fax/hand-deliver/mail the forms to them.

And yeah, it turns out faxes are a dollar a page at my convenience store.
