It depends on the med school. Most don't require a full course load each year, but many will offer alternate GPA calculations if you do have a full load each year. For example, UofT allows students with a full course load each year to drop their lowest full-term equivalent (6 units) from each year, which is the equivalent of two 3-unit courses per year at McMaster. If you don't have a full courseload even one of those years, you don't get to drop any marks from the GPA calculation.
You don't
have to drop a course, you
want to for whatever reason. You could, however, take another course next term to make up for it and still have the full courseload over the year. Taking courses over the summer will not count towards a full courseload for the fall/winter term. Look into the specific schools you want to apply to and see what their rules are, particularly if you're looking outside Ontario.
