Yeah, I've yet to fill out my new student form, I'm not even sure I can find the sheet. I am going to Mcmaster on the 19th and I was wondering where i could pick up this form to fill out and where would I hand it in?

You'd need to hand it in to the Office of the Registrar  (look at the top right hand corner of the link jhan has )

Does the background of the photo that you are sending with your form have to be white? Thanks!

I was planning on stopping by to drop off my student form tomorrow. Am I able to have my photo taken at Mac at any time or is there a set date for when that is available? Also, where would we get our photo taken? Thanks!

You need to go to the registrars to have your photo taken. You can go anytime they are open, here's their contact info:

Was the student form supposed to be in the acceptance package? If so, I haven't received the form in my acceptance packages.

What about international students? Do we have to mail this in as well? Will the card be mailed back to a location outside canada.? Is it worth or is it better to get it during the welcome week?

I am not entirely sure, but feel free to contact them and ask. Here's their contact info:

Their email is found at the bottom of the page.
If they don't reply, you can give them the new student form along with the pic when you come to canada. They can make your student card on the spot or tell you to pick it up after coupla days.
