Immunization For Nursing
Hi, i want to know whats involded n where u should do the immunization. do we need to take forms 4 the doc tp fill???? am soo lost

pretty sure you can see your family doctor and get it done.

if you have the paperwork that needs to be filled in,
take it to your family doctor.

they will write you a requisition to go get blood taken, then you just go to your nearest hospital or clinic that has a lab (your doctor will tell you where to go... or just take the blood themselves...)

they're testing to check to see if the immunizations you got when you were younger are still in your system. if they come up negative, you'll most likely have to get new shots....

and depending on your doctor it can cost $15- $40+ for them to fill the forms out for you.. so bring cash

you should be up to date with all your immunizations if you live in ontario and schooled here....

Requests for checking if you are immune are ordered by the doc on campus or you could even go through your family doc. I'm in the momac program so we have our own doctor...im not sure how it works for mac fully. Anyways then you go and get some blood work done and the results are handed into the nursing dept. If you are not immune for some reason or have not been vaccinated to begin with you'll have to get the shots and have more blood taken at a later time to check for antibodies.

The other option (to avoid blood work) is to call the public health department and get your vaccination record faxed to your fam doctor or mailed to you and present that to your department.

PS: are you just entering the program or moving into second year?

we dont ave any paperwork for the doc to fill send.....
ave lived in Canada 4 jst a yr
ave now entering 1st yr......

The form consists of at least 6 pages. The first section is to be completed by the student. It's basically just a checklist of things to get done. Section 2 is done by the office so that you can get cleared to the program. If you're not cleared the form is sent back to you, and you have to make sure everything is done, or else you don't continue.

Section 3: tetanus/ diptheria. you have to get a shot within ten yr. span. If it's been that long, you have to get a new one.

Section 4: Measles, Mumps, and Rubella. you have to be immune with these three, and this is done through blood titres. If one's not immune, they give an MMR vaccination Varicella. If blood tests show that you're not immune, you have to take 2 vaccinations. The second one has to be at least 28 days after your first shot.

Section 5: Hep B. Basically, if you're not immune, you get three shots. In order to get cleared into the program, you have to at least have done two shots out of three. Your last shot can be done after you have been cleared and the paper is given back to you. (Make sure you bring your paper back though, after completing it). If in any way, you're still not immune after those 3 shots, you have to complete Part A or Part B (whichever one applies to you).

Section 5: TB Screening. You have to get a TB shot (I think this is 42$ now) and if it reads positive, you have to go and get a Chest Xray.

Section 6: very straight forward, get all your HCPs who have participated in your immunization to sign it.


P.S. I do believe there's a cost in HepB shots. I just had to take HepA and HepB so the price of that is a bit different. Just ask an HCP, and they can help you with that.

P.P.S. And yes, it is better to take the form with you every time you see the doctor. That way, you can keep track of things that have been done, and things that have yet to be done! Good luck! : )

You should've gotten it by mail. If in any case you haven't, just call up the Nursing office, so you can clear things up with them! : )

i got my nursing package today. there is a 3 page form in which the doctor has to fill up and if u have any missing immunizations that is needed, u 'll have to make appointment to ur doctor.
