Name on graduation
Does every graduate's name get published in the convocation handbook or only the ones attending the convocation ceremony? Is there an option to opt out of this and not publish your name?

Your name will be listed regardless of whether or not you attend the ceremony. Your name will also be listed if you get the senate scholarship (or any other scholarship/award, no matter how random), regardless of whether you're graduating or not - ie students from all years/faculties who get a scholarship/award have their names listed in the program.

Edit: You *can* opt out. It says at the bottom of the graduation ceremony program, "Some names of graduands may not be included in this program at the request of these individuals."

I'm pretty sure it's something that you have to opt out of, rather than "opt in" to get your name in the program. It's automatically there and you need to contact someone (the registrar?) to remove it.
