I've been in contact with Pauline Taggart to get a suggestion form on the MSU site. Sadly, I have zero skills when it comes to web-based stuff and basically broke the computers. she's still fixing them.
My goal for the form is much more simple than that survey, but it would be a place for students to anonymously submit both good and bad comments about the MSU and its services etc.
To those of you who have reservations about submitting your feedback to random people on the internet, I implore you to talk to your SRA members.

You don't submit any of your information in the survey, if that's what people are worried about. The most identifying information you give is your faculty and year...

As the Services Commissioner, I am really interested in getting their short answer questions 1 and 3 to help guide some similar discussions the Services Committee is having this year. I know you can't tell me who this is - but is it possible for you to contact them and ask if I can get some of that feedback when they are done, whether it be through you or an anonymous e-mail?
