Reinstatement questions
I've tried to find some recent threads to add my reinstatement questions to but the ones I found were from 2012 and were closed so I thought I'd start a new one.

1. When getting the actual paper application do I need to show anything at the administration office like my student card or any identification with me?

2. Can anyone just go in the admissions office and pick up a reinstatement application, even for someone else?

I was going to ask about this form I saw online too but since I'm a new member I couldn't post any links. So it's a legit form?

Since I'm trying to gain re-admission via a reinstatement application I'd like to confirm if I will be needing apply via OUAC at any point?

I ask because if I get re-admission via reinstatement application it appears I'd save a lot more money-not to mention save myself from a walk of shame to the admissions office if the above form is the authentic form. A reinstatement application is a $ 75 fee wheras having to apply via OUAC will cost hundreds of dollars.

No. Simply fill out the form and return it along with your letter of explanation/documentation/new grades, etc. You will pay the $75 fee at that time and then that's it, no OUAC.

Walk of shame? Better not reflect this attitude in your reinstatement letter. You always learn something from a failure. Wear it as a badge of honor.

Strange though...the first page of the application says "Application Fee: $100" at the very top then at the bottom of the page to attach a $75 fee. So which is it?

I have some further questions regarding reinstatement.

Does it have to be in the same program you could not continue in or could it be in another program so long as you have qualifications?
